Working with the patient’s physician, getting well at home is possible with an interdisciplinary home health care team of visiting nurses, therapists, caregivers and social workers. VNA Hospice & Palliative Care is a non-profit provider of comprehensive care and support to patients and families facing the challenges of life-limiting illness and loss. Care is coordinated among a team of clinicians including medical doctors, nurses, hospice aides, social workers, chaplains and trained volunteers. Support can take many forms; medical, physical, emotional, practical and spiritual, ensuring help is provided to all involved. Many patients receive hospice care in their homes, whether that's a private home or apartment, or in a nursing home or skilled nursing facility.
We also work closely with referral sources in Iowa to assist clients in need. VNA staff work with Jolene regularly to help her maintain her overall health and well-being. VNA delivers expert, compassionate care to under-resourced individuals and families in our community. Services given to help you return to usual activities after illness either on an inpatient or outpatient basis. Treatment of injury and disease by mechanical means, like heat, light, exercise, and massage. 2 Agencies that are Medicare Certified are able to participate in Medicare related programs.

Physical Therapists assess your muscle strength, joint movement, balance and mobility and provide exercise programs and instruction to restore and enhance independence and safety. Occupational Therapists provide instruction and training to help you improve or regain independence with daily activities such as bathing and dressing.

A newsletter from PBHHH arrived in my email announcing training for hospice volunteers. My family had experienced the need for hospice twice, once in Ohio and again in Florida. The relief my family and I felt when the hospice volunteers arrived was beyond measure. PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.

This care is in conjunction with active treatment for a disease. The palliative care team focuses on the pain and discomfort a patient may feel while undergoing treatment while your primary doctor and specialist focus on treating your illness. While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health since October 2021. When I retired, I began searching for volunteer work that would allow me to give back to my community.
